Open Heel SMO for Children in Hamilton
Surestep Support With the Heel Left Open
The Open Heel SMO gives the same close, steadying support as the standard Surestep SMO, but with the heel area left open. For some children this makes the brace more comfortable, cooler, or easier to accept.
At Foot Foundation in Hamilton, we assess your child first, then recommend the version that best suits their feet, their comfort and their daily life.
What Is an Open Heel SMO?
An SMO (supramalleolar orthosis) is a thin, flexible brace that wraps the foot and finishes just above the ankle bones. The Open Heel version leaves the back of the heel exposed rather than fully enclosed.
It is a Dynamic Stability brace, so it steadies the foot and ankle while still allowing plenty of natural movement.
Which Children Might It Help?
An Open Heel SMO may be considered for children who:
have feet that roll in when standing or walking (pronation)
have low muscle tone and tire easily (hypotonia)
have feet and ankles that wobble in several directions under weight
are not yet standing independently
walk on their toes a little (mild toe walking)
It is designed for children up to about 36 kg.
Why Choose the Open Heel Version?
We may suggest the open heel design when:
your child's heel gets hot, sweaty or irritated in a closed SMO
your child is sensitive to a fully enclosed feel around the heel
the heel skin needs to be checked or kept free from pressure
your child needs a little more freedom of feel at the back of the foot
The support at the foot and ankle stays the same. What changes is the coverage at the heel.
Not sure which SMO suits your child?
What to Expect
Assessment: we check walking, foot shape, flexibility, muscle tone and balance.
Measuring: if an Open Heel SMO suits, we capture your child's foot shape.
Fitting: we check comfort, heel position and shoe fit, and watch your child walk.
Break-in: short wearing times to start, building up over a week or two.
Reviews: regular checks as your child grows and changes.
Everyday Life With an Open Heel SMO
Shoes: a supportive shoe with a wide opening, often half to one size bigger.
Socks: a thin, snug sock helps prevent rubbing.
Skin checks: mild redness that fades in 20–30 minutes is normal. Lasting marks or blisters need adjusting.
Cleaning: wipe with a damp cloth, air dry away from direct heat.
Growth: children outgrow braces quickly, so keep review appointments.

Frequently Asked Questions
Does an open heel mean less support?
No. The foot and ankle support is the same. The open heel changes coverage and comfort, not the level of stability.
Will it stay on properly?
Yes. It is custom-made to your child's foot and secured with straps, then worn inside a supportive shoe.
Can my child wear it with any socks?
Thin, snug, seam-free socks work best. Thick or bunched socks can cause rubbing.
Can my child wear it at kindy or school?
Yes. Once it is broken in, most children wear their SMOs through the day.
How often will it need replacing?
It depends on growth and wear. Many children need a new size every 9–18 months, which we monitor at reviews.
